What types of health insurance do you accept?

While we don’t directly accept insurance, most medical insurance plans provide re-imbursement directly to you for our services.  We can provide you with all necessary documentation to submit to your insurance and provide some guidance to help you through that process.

Do I need a doctor’s referral?

No and Yes.  California law does not require a referral for physical therapy.  However, a referral from your medical provider can be helpful for insurance purposes.  Don’t worry if you don’t have a referral to get started, we can help you get one if needed.

What are your rates?

  • In-home appointments:  $175
  • In-clinic appointments:  $135
  • **We request 50% deposit at the time of scheduling to reserve your appointment time.  Deposit is non-refundable with cancellations with less than 24 hours notice **

How am I protected from surprise billing?

By having a fixed price for services, there is never . . . NEVER a surprise.  You know the cost of each session when you make your appointments and that number never . . . NEVER changes!

Why would I use an out-of-network therapy service?

Using an out-of-network provider doesn’t have to be a scary process and many health insurance plans have out-of-network benefits for their subscribers.  Here are some reasons why people choose to go out-of-network:

  • You want control over your healthcare decisions   using out-of-network providers takes control back from your insurance company and directs it towards your choices.
  • Privacy   an out-of-network provider is not required to share your information with your insurance company.  Forza always protects your information according to HIPAA guidelines and only shares it with whom you designated or where required by law.
  • You don’t want to wait to get started   you don’t have to wait for insurance authorization when using out-of-network providers.
  • You have good out-of-network benefits —  many insurance plans will cover medically necessary services to out-of-network providers, sometimes with little difference in out-of-pocket costs from in-network providers.  Sometimes out-of-network costs can be applied to your in-network deductible. 

Price Transparency

Starting January 1st, 2021 The Center for Medicare and Medicaid services mandated that hospitals make public their standard charges for hospital services.  The table below shows the standard charges two local hospitals use for a few common physical therapy services.  The First day of PT and subsequent visits are estimates of the hospital charges a patient might see on their bill from the hospital and the total may vary based on the charges entered for each treatment.

What is your new client procedure?

After your first visit has been scheduled, our therapist will contact you to get brief history of your case.  We will also email typical medical paperwork (detailed history, consent, etc.) ahead of time. This will help to make your first in-person visit more efficient, allowing for the most time to be spent hands-on.
